Transition Services-Leaving School
Neighbourhood Connections has a contract with the Ministry of Social Development to provide Transition Services to students with a disability who are leaving school.
This service can be provided to students who:
Are currently living in New Zealand and are a New Zealand Citizen or permanent resident.
Are aged between 16 and 21, receiving ORRS funding and are in their last year of school.
Aims of the service:
• Help students think about what they might like to do when they leave school.
• Support them to make contact with other agencies or community services.
• Coordinate a plan to assist the student to achieve their goals.
How can I access this service?
You or your school can make contact with us directly, we will arrange to meet and discuss further what our organisation can do.
If you want to go ahead we have to contact the Ministry of Social Development to confirm that you are receiving ORRS funding.
Once we have confirmed this we will meet with you and someone from your school to sign an agreement to work together.
What happens next?
We will get together with you, your family/whanau, teachers from your school and anyone else who you might want to invite to make a plan about what you want to do when you leave school.
This is your plan so we will support you to make choices about what you want by sharing information with you about activities and services that are in the community.
We will talk with you and your school about when you can go and visit some of the places and services in the community and who will support you.
We then continue to meet with you, your family/whanau, and your teachers during the year to see if the things that are in your plan are happening.
